application & order form below (2 pages), or contact us to have one faxed or emailed you. Please allow 1 full
business day for your application to be processed, and we will contact you to confirm your order and issue your
individual customer number. Future orders can be placed via phone, fax, or email. If you are ordering by fax or
email, please remember to include your customer number, item number with detailed description, product quantity,
shipping preference, and method of payment with an updated, accurate shipping address. A $25 minimum purchase is
required for each order (not including shipping charges).
